New Patient Info
You will be required to complete a Patient Registration Form and a Patient Health History Form before your appointment. Save yourself time at check-in by completing all necessary information and checking in ahead of time on our patient portal. If you do not use the portal, plan to arrive at least 15 minutes early for your appointment.
Please remember to bring:
Your previous health provider's contact information so that we may request records. It is very important that we have a complete understanding of your health history in order to provide the best possible care.
If you would like to transfer your records in advance, have your current provider fax records to Dahl Memorial Clinic, attn Reception at 907-983-2793 or mail to:

2. A list of your current medications
3. Your insurance card
4. Your photo ID
Most office visits take 60 minutes, however your first visit may take longer in order to address your immediate medical concerns and to discuss your health history and health goals.
Scheduling Appointments
Please call 907-983-2255 to schedule an appointment or request an appointment time through our patient portal. We do our best to accommodate same day appointment requests. Please arrive on time for your appointment.
Cancellations. Please call in advance if you are unable to come in for your appointment. You are considered a no-show if you do not notify us in advance of missing your appointment. A $20 fee will be applied for no-shows. After 3 no-shows, you will no longer be able to make appointments and will be seen on a walk-in basis only for the rest of the calendar year.
Urine Drug Screens. We must have a screening referral form and a photo ID from the patient prior to conducting the screening. We accept appointments for urine drug screening beginning at the following times:
Tuesday from 9am to 1:30pm
Wednesday from 9am to 11:30am
Thursday from 9am to 1:30pm
Department of Transportation Physicals. This is an hour long appointment. Please plan on arriving at least 15 minutes early. You must bring your valid driver's license to your appointment. Note: DOT Physicals are not covered under DMC's Sliding Fee Scale.
Behavioral Health Appointments. These are scheduled through the front desk. Please plan on appointments lasting about 60 minutes.
Fees and Billing
Dahl Memorial Clinic's fees are set biennially by the board of directors. Our fees are set to be comparable or slightly lower than the fees of other facilities our size in our geographic area.
We bill all private insurances, Veterans Administration, Medicare, and Medicaid. Please make your co-payment at the time of your visit.
We serve all patients regardless of ability to pay. If you do not have insurance and cannot pay your balance, please speak with our Billing Specialist about establishing a no-interest payment plan.
